ALBANY β€” The state's inspector general says New York City's Office of Chief Medical Examiner relied for a decade on substandard work of a lab criminalist before she was suspended from casework in early 2011.

State Inspector General Catherine Leahy Scott says the analyst had consistently poor job reviews and repeatedly failed part of the exam for her work title.

The report says errors by the analyst and the misplacement of items in her cases went undetected for years, but after her suspension, the office quickly reviewed her work and ensured corrective actions were taken.

The office agreed to require employees to pass all aspects of their training program, ensure no criminalist has more than one case open for examination at a time and ensure supervisors have access to prior work evaluations.
